Abstract

Calcium channel blockers (CCBs) are widely prescribed for cardiovascular conditions and are increasingly recognized for their off-label uses. However, their association with lower urinary tract symptoms (LUTS) remains underappreciated in clinical practice. This narrative review explores the mechanistic pathways through which CCBs contribute to urinary symptoms, including their natriuretic and diuretic effects, anticholinergic activity, alpha-adrenergic receptor blockade, and direct inhibition of detrusor smooth muscle contraction. Recent studies, including data from Pakistani populations, reveal a significant association between CCB use—particularly non-dihydropyridines and certain dihydropyridines—and the onset or worsening of LUTS. These symptoms adversely impact patients’ quality of life, sleep, and mental well-being. Increased awareness among healthcare professionals is critical to avoid misattribution of symptoms to aging and to guide appropriate therapeutic adjustments, such as switching to more vascular-selective CCBs. Understanding and addressing this adverse effect may prevent unnecessary polypharmacy and improve patient outcomes.
