Objectives: Patient satisfaction is a key indicator of healthcare quality and reflects how effectively perioperative care meets patients’ expectations and informational needs. The perioperative period is often associated with heightened anxiety, making the quality of anaesthesia evaluation and communication particularly important. Understanding factors associated with satisfaction can help improve patient-centered anaesthesia services. Methods: This cross-sectional study included 155 adult patients undergoing elective surgery at a tertiary care hospital in Karachi. Patient satisfaction with perioperative anaesthesia evaluation was assessed using the Leiden Perioperative Care Patient Satisfaction Questionnaire. Demographic and socioeconomic data were collected through structured interviews. Associations between satisfaction and participant characteristics were analyzed using chi-square or Fisher’s exact tests, with statistical significance set at p ≤ 0.05. Results: Overall, 116 participants (74.8%) reported satisfaction with their perioperative anaesthesia evaluation. Satisfaction was significantly higher among patients aged 46–70 years compared with those aged 18–45 years (81.3% vs. 60.4%, p = 0.01). Educational status was also significantly associated with satisfaction (p = 0.01), with the highest satisfaction observed among participants with no formal education and those with higher education. No significant associations were identified for gender, place of residence, occupational status, or family monthly income. Conclusions: Nearly three-quarters of patients undergoing elective surgery were satisfied with their perioperative anaesthesia evaluation. Age and educational status were significantly associated with satisfaction, highlighting the importance of tailored perioperative communication strategies to enhance patient-centered anaesthesia care.
