Objective: To evaluate the association between HDL-C levels and in-hospital outcomes, including mortality, reinfarction, heart failure, and stroke, in STEMI patients treated with primary PCI, with a focus on reassessing its role in risk stratification given recent questions about its predictive value. Methodology: A total of 300 STEMI patients were enrolled and divided into two groups based on their HDL-C levels: low (<1.04 mmol/L) and normal (≥1.04 mmol/L). Medical records were retrospectively reviewed, and Chi-square testing was employed to evaluate the relationship between HDL-C levels and in-hospital outcomes. Multivariate adjustments for potential confounders were not included in the analysis. Results: In the low HDL-C group (n=150), 12 patients (8%) died, while 10 patients (6.67%) in the normal HDL-C group (n=150) experienced in-hospital mortality (p=1.0). Reinfarction occurred in 8 (5.33%) patients in the low HDL-C group and 6 (4%) in the normal HDL-C group (p=1.0). Heart failure was noted in 15 (10%) patients with low HDL-C and 14 (9.33%) with normal HDL-C (p=1.0). Stroke occurred in 7 (4.67%) and 6 (4%) patients in the respective groups (p=0.68). These findings suggest no significant association between HDL-C levels and in-hospital outcomes. Conclusion: HDL-C levels were not significantly associated with in-hospital outcomes such as mortality, reinfarction, heart failure, or stroke in STEMI patients treated with PCI. This study highlights the need for further research incorporating HDL functionality and other biomarkers, such as the LDL/HDL ratio, to better predict clinical outcomes in this patient population.
